Abstract

¸ñÀû: º» ¿¬±¸ÀÇ ¸ñÀûÀº º´µ¿¿¡ ÀÔ¿øÇÑ ¼ºÀΠȯÀÚ¸¦ À§ÇÑ Çѱ¹Çü ¼ºÀΠȯÀÚ ¼¶¸Á ¼±º° µµ±¸(K-APDS)¸¦ °³¹ßÇÏ°í, ½Å·Úµµ¿Í Ÿ´çµµ¸¦ °ËÁõÇÏ´Â °ÍÀÌ´Ù.

¹æ¹ý: µµ±¸ °³¹ßÀ» À§ÇØ ¹®Çå°íÂû °á°ú¿Í ÀÏ¹Ý º´µ¿ °£È£»çµéÀÇ Æ÷Ä¿½º±×·ì ÀÎÅÍºä °á°ú¸¦ ÅëÇØ 12°³ ¹®Ç×À» µµÃâÇÏ¿´°í, Àü¹®°¡ ³»¿ëŸ´çµµ¸¦ È®ÀÎÇÏ¿´´Ù. °³¹ßµÈ µµ±¸ÀÇ ½Å·Úµµ¿Í Ÿ´çµµ °ËÁõÀ» À§ÇØ 2022³â 10¿ùºÎÅÍ 11¿ù±îÁö »ó±ÞÁ¾ÇÕº´¿ø 3°÷ÀÇ º´µ¿¿¡ ÀÔ¿øÇÑ 317¸íÀÇ ¼ºÀΠȯÀÚ¸¦ ´ë»óÀ¸·Î ´ã´ç°£È£»ç°¡ Æò°¡ÇÏ¿© ÀÚ·á¼öÁýÇÏ¿´´Ù. ¼öÁýµÈ ÀڷḦ ÅëÇØ ±¸¼ºÅ¸´çµµ, ´ëÁ¶±×·ì Ÿ´çµµ, ÁØ°ÅŸ´çµµ, ¿¹ÃøŸ´çµµ ¹× µµ±¸ÀÇ ½Å·Úµµ¸¦ ºÐ¼®ÇÏ¿´´Ù.

°á°ú: ±¸¼ºÅ¸´çµµ °ËÁõÀ» À§ÇÑ ¿äÀκм® °á°ú 2°³ÀÇ ¿äÀÎÀ¸·Î ÃßÃâµÇ¾ú°í, Àüü º¯·®ÀÇ 60.1%¸¦ ¼³¸íÇÏ¿´´Ù. ´ëÁ¶±×·ì Ÿ´çµµ °ËÁõ¿¡¼­ ¼¶¸Á±º°ú ºñ¼¶¸Á±º °£ÀÇ K-APDSÀÇ ¼¶¸Á ¹ß»ý Á¡¼ö Â÷ÀÌ´Â ¸Å¿ì À¯ÀÇÇÏ°Ô ³ªÅ¸³µ´Ù(Z=-10.82, p<.001). ÁØ°ÅŸ´çµµ °ËÁõÀ» À§ÇØ K-APDS¿Í Delirium Observation Screening°ú Pearson's correlation coefficient¸¦ È®ÀÎÇÏ¿´°í .94 (p<.001)·Î ³ªÅ¸³µ´Ù. ¿¹ÃøŸ´çµµ °ËÁõ °á°ú ¹Î°¨µµ´Â 91.1%, ƯÀ̵µ 82.4%, ¾ç¼º¿¹Ãøµµ 52.6% ¹× À½¼º¿¹Ãøµµ´Â 97.8%·Î ³ªÅ¸³µ´Ù. K-APDSÀÇ ½Å·Úµµ´Â Cronbach's ¥á=.91·Î ³ªÅ¸³µ´Ù.

°á·Ð: K-APDS´Â 2Á¡ À̻󿡼­ ¼¶¸Á ¼±º°ÀÌ °¡´ÉÇϸç, ¿ì¼öÇÑ Å¸´çµµ¿Í ½Å·Úµµ°¡ °ËÁõµÇ¾ú´Ù. µû¶ó¼­ º» µµ±¸´Â ÀÓ»óÇöÀå¿¡¼­ ȯÀÚ¸¦ °£È£Çϸ鼭 ¹Ù·Î Àû¿ëÀÌ °¡´ÉÇÏ°í ¼¶¸Á Á¶±â ¹ß°ß ¹× ºü¸¥ ÁßÀç Á¦°ø¿¡ ±â¿©ÇÒ °ÍÀÌ´Ù.

Purpose: The purpose of this study was to develop a Korean Adult Patients Delirium Screening Tool (K-APDS) for those admitted to general wards, and to verify its reliability and validity.

Methods: For the development of the tool, 12 items were derived through the results of literature review and focus group interviews with general ward nurses, and the content validity was confirmed by experts. To verify the reliability and validity of the developed tool, 317 adult patients who were admitted to general wards of three tertiary general hospitals from October to November 2022 were evaluated by the attending nurse and data were collected.

Results: After factor analysis for construct validity verification, two factors were extracted, which explained 60.1% of the total variance. After the validation of the control group, the difference in the delirium incidence scores calculated using the K-APDS between the delirium group and non-delirium group was very significant (Z=-10.82, p<.001). To verify the criterion validity, K-APDS, Delirium Observation Screening, and Pearson's correlation coefficient were checked and found to be .94 (p<.001). The predictive validity test reported that the sensitivity was 91.1%, specificity was 82.4%, positive predictive value was 52.6%, and negative predictive value was 97.8%. The reliability of K-APDS was found to be high with Cronbach¡¯s ?=.91.

Conclusion: K-APDS can screen for delirium with 2 or more points, excellent validity and reliability have been verified. Therefore, this tool could be applied immediately in the clinical field, and will contribute to the early detection of delirium, enabling rapid interventions.
